NURSE'S CORNER

 

• Immunization Letter: Provides updated information about required immunizations required for school admission.

• LA Immunization Requirements: A copy of the Louisiana Statute concerning immunizations.

• Medication Letter: Information about having your child's medication given at school.

• Parent Authorization to Administer Medication: Paperwork giving the nurse permission to administer medication.*

• Doctor's Order for Medication:  This paper work is to be completed by the doctor.*

• School Health Rules:  General information about health rules for the school.

• Please let Nurse Jenafer or Nurse Cindy know if there is any change in your child's medical condition.

 

* If your child requires medication to be administered during school hours,  both of these forms must be completed and returned to the school nurse before medication can be dropped off or administered.
